Prepare Insurance and Financial Information Before Your Visit

Healthcare billing can be confusing, but we want to help you understand the process. We will bill your insurance provider, but the balance of the bill will be your responsibility. To make sure this process goes smoothly, it is very important that you bring the right information with you on the day of your visit. If you do not provide your health insurance information, we must bill you as a self-paying customer until we receive this information.

Please prepare to bring with you on the day of your visit:

  • The patient’s legal name, date of birth, home address and name of primary care doctor.

  • The guarantor is the person financially responsible for the child’s medical bills; please bring the guarantor’s legal name, ID card, mailing address and phone number.

  • The guarantor’s health insurance card containing policy numbers, insurance claims address and customer service phone number.

  • The copay amount due for the child’s visit and procedures.

  • Parents’ or legal guardian’s names, dates of birth and social security numbers.

  • The names of the parents’ employers and work phone numbers.

Insurance Pre-Approval

Your insurance provider may require that your child’s hospital stay be approved before she arrives for her visit. The doctor who referred your child to Children’s for her visit should handle these approvals. However, you should call that doctor’s office or your insurance provider to make sure approval has been granted; otherwise, you could be held financially responsible.

Co-Payment for Services

At the day of your child’s visit, you will need to bring payment to cover any co-payment fees associated with your insurance.

The Children’s Hospital accepts the following types of payment:

  • Cash (in person only)

  • Personal Check or Traveler’s Check

  • Money Order

  • Visa

  • MasterCard

  • Discover

  • American Express

Financial Assistance

If you do not have health insurance or other financial assistance, please call 720.777.1234 and ask for Financial Aid. We can help determine eligibility for financial aid programs that are available to you.

Coming to Children's?

Our visitation policy is changing on October 1, 2009.

For the health of our patients, sick visitors and children under age 12 are not allowed in inpatient areas.

Please familiarize yourself with our current visitation restrictions before your visit.
