Continuing Education Memorandum to Fulfill ACCME Essentials – This memo outlines faculty requirements to Fulfill CME Essentials and Standards for a CME activity.

Faculty Full Disclosure and Conflict of Interest (COI) Resolution Form   – Disclosure of Relevant Financial relationships is required by all faculty who present at certified CME activities.   Disclosure must be signed prior to the activity.  Faculty who identified a relationship must complete the COI form.  Faculty who refuse to sign the disclosure are eligible for disqualification from the activity.
