What is Scoliosis?

Scoliosis is a lateral curvature and rotation of the spine. It often appears in association with growth spurts. Scoliosis can occur in otherwise healthy individuals or can be associated with other diagnosis, such as Cerebral Palsy, Spina Bifida, Duchenne’s Muscular Dystrophy, and Neurofibromatosis; or associated with congenital abnormalities of the spine. In otherwise healthy individuals, its occurrence is approximately 2.5/100 children. There are varying levels of severity: less than 10 degrees is considered normal, between 10 and 25 degrees is considered mild, 25-40 degrees is considered moderate, and greater than 40 degrees is considered severe.

Treatment of Scoliosis

Because scoliosis changes with growth, it is important to monitor the progression closely. Radiographs two or three times per year through growth are sufficient to monitor progression of scoliosis. Curves less than 25 degrees are usually observed closely with no treatment. Curves between 25 and 30 degrees with the apex of the curve in the thoracic vertebrae, or quickly progressing curves can be treated in a brace. The brace is called a TLSO (thoraco-lumbar-sacral orthotic). It extends from just under the arms to over the pelvic bones. It is designed to be worn for 18 hours per day. It is custom molded to provide a perfect fit and provide opposing pressure against the scoliosis.

Curves greater than 40 degrees: Have a significant risk for further progression after growth has finished and could require surgical intervention. Surgical options include anterior or posterior approach. Goal of surgery is to decrease size of curve and fuse the multiple vertebrae into one solid bone to eliminate the progression of the curve.
